You have been asked to develop a safety case for an FPSO facility. A formal safety
case requires the following:
• A description of the facility and the operating environment
• Formal Safety Assessment
• Safety Management System with ALARP approach to risk assessment
Based on your knowledge of Risk Management and Safety Case, outline the
approach that you would use to develop the safety case.
This assignment is to be submitted at the latest by Friday 26th September, 15.30
hrs, and prior to the Semester Test.
The format should follow ECU protocol and be submitted on plain A4 paper with a
cover sheet. No informal submissions will be acceptable, i.e. on scratch pads or
rough work sheets, and no snapshot images of written material will be accepted. If
submitting by softcopy (email, etc.) the document shall be printed or written out
on plain A4 white paper before being scanned for submission as a .pdf file.
This assignment forms an important part of Unit ENS2180 and will carry 20% of
the final mark for this unit.As part of the required learning outcomes of this Unit you are required to access risk
assessment information for an offshore structure and develop a safety case in compliance
with current offshore regulations.

Guidelines to preparation of a hypothetical Safety Case
1. Introduction should describe / define what a safety case is, and what the object of
the safety case is.
2. Prepare a Table of Contents. The Safety Case will be presented in a Safety case
document called a Safety Case Report.
3. Describe the facility as well as possible, and make reasonable assumptions where
there is a lack of detail. State assumed operational aspects and other attributes of
4. Identify HAZID (Hazard Identification) risks – these are hazards that can affect the
facility externally to the operational processes, i.e. they do not arise from the
primary function and processes on the facility, hence they are not an intrinsic part
of the facility operation. Some examples of HAZID are extreme weather, physical
condition and deterioration of the FPSO structure due to corrosion or environmental
loading, fatigue, boat operations, helicopter activity, etc. You need to identify other
HAZID items and compile a list that will form input to the Safety Case process. This
is an important part of the exercise.
5. Identify HAZOP (Hazard and Operability) risks – these are risks that are intrinsic to
the day to day operational processes on the facility, such as gas or oil leaks,
mechanical, electrical, or structural failure of the primary process equipment or
systems relating to the operation; cargo transfer to external receiving vessel; any
risks that can give rise to explosion or fire. The list needs to be developed more to
be as exhaustive as possible. Remember that your safety case is like a legal
argument, and if you do not cover all reasonably identifiable risks your safety case
argument for safe operability will FAIL!
6. The risks need to be organized in a manner to help the assessment process and
grading of the risks – this process should qualify and quantify the risk levels and
the consequences to human life, the environment, health and monetary value of the asset and revenue. Refer to the example documents and official guidelines that
are available from the blackboard. Use the more simple tools in the Guidelines to
assess each of the risks in turn, and determine the level of potential risk of each of
