This is an essay where a student Nurse(SN) need to assume that he/she have interviewed 40 year old male Ashu (client). He is from Nepal. He is a Hindu. He has been in Australia for 20 years now. He has a chronic illness i.e Asthma . SN need to discover the impact this has had on their Quality of life, ability to function,the way they set up their daily activities and the impact on life of their family, and to identify care priorities/areas of need.
you can make up things about his health status and other personal background as required. student need to Present this paper based on the Roper, Logan & Tierney (Rlt) (2008) model for nursing as a framework and use the first 5 stages of Levett-Jones (2018) Clinical Reasoning Cycle to identify care priorities. It is expected that appropriate evidence-based literature will be used to support the essay.
This assignment (essay) is based on interview performed by student nurse on a person about how their chronic condition impacts their ADLs (RLT MODEL) and quality of life then Collaborate with them to establish Smart Care Goals for their management of their condition. Care priorities that they see for themselves and sits within the impact of their disease on them. Things they might want to change. What are things that they might want to change in their circumstances (SMART). A kind of motivational interview would be good.
The goals set for this client must be smart i.e specific, measurable ,attainable, relevant and time- based eg : pain management. You can’t set goals that is not achievable for the client.
Student nurse should link underlying knowledge/ research of the pathophysiology into the written discussion using CRC to make sense of symptoms/situation BUT the focus is on the overall impacts to the individuals life.
There are chances that all the aspects of RLT model might not be covered so it has to be made clear in the introduction part.( Eg:- Not all of the aspects in RLT model will be covered in this discussion because it didn’t came up in the conversation with my client)cdk-8.pdf.aspx?inline=true
