1 This is my tutor feed back: Structure The document was organized with good use of headings. You used consistent formatting throughout, but I did find one heading that was too long. You might want to consider rephrasing it to make it more concise. I thought the introduction was clear and it let me know what to expect in the sections below. Unfocused paragraphs: Your paragraphs should be as focused as possible. Currently, there are some instan ces where the paragraph loses focus. When writing paragraphs, ensure you have one main idea per paragraph. This means that the first sentence of your paragraph indicates the controlling idea of the paragraph, and the rest of the information in that paragra ph is related to it. We recommend Choice of language This was one of your weaker areas. I found numerous instances of unclear and informal language, clunky wording, and incorrect word choice. I also flagged a number of unintroduced acronyms. Informal language: To improve the overall quality of your writing, you will need to use more sophisticated/academic wording. For instance, rather than writing 'give' try using a word such as 'provided'.These simple changes can have a very positive impact upon the quality of your writing. Sometimes the way ideas are expressed may be unclear. This c an be the result of poor word choice or wordorder or the ideas are confused. It is important to read over your work carefully, preferably aloud, todetermine if the meaning is clear. Clarity: Using more words than are necessary to convey our message can mak e the sentence/paragraph quitewordy and confusing. Using more clear and concise language will eradicate this issue and will improve theoverall quality of your writing. Acronyms: Spell out the words on first use, put the acronym in parentheses, then use the acronym for allsubsequent uses. For example, Royal Society for the Prevention of Cruelty to Animals (RSPCA). Unnecessary language: Some people believe that good writing is characterised by highly complex languageand sentences. In fact, this is not the cas e. It is better to use simple language and sentence structures tocommunicate ideas, being mindful that language must be appropriate for the context and audience. By usingprecise words and eliminating wordiness and redundancy (unnecessary repetition), your writing will be clear, concise and easy to understand. For example, 'The investment had some costly results' is much more 2 concisethan 'the investment had some bad results that cost the firm a lot of money'. To eliminate wordiness, try andsubstitute precise words for long phrases. Incorrect word choice: To improve the clarity and quality of your writing, ensure that you are using the mostappropriate words within each context. Incorrect word choice can significantly hinder the meaning for areader. Repetition: Be mindful of using similar language too frequently. In a highlighted instance of your text, youhave included the repeated words quite close together. An alternative wording choice may be required tominimise reader confusion and ensure a more natural flow. Spelling / Grammar I have flagged a number of errors in this area. I found instances of missing definite and indefinite articles, commas and colons. I have also highlighted run -on sente nces and incorrect comma usage. Indefinite articles: In the fo llowing sentence, the indefinite article 'a' is used before the noun 'pen'. In thiscase, it it used because it is referring to a pen, but does not need to specify which pen. E.g. 'The teacher gaveme a pen.' For nouns that begin with a vowel (a, e, i, o, or u), you would use the indefinite article 'an'. E.g. 'Theteacher gave me an apple. Run -on sentences: A run -on sentence occurs when a sentence combines two or more independent clauseswithout using appropriate punctuation or a conjunction to separate them. F or example: ‘The research wasconducted by an in -depth survey the results were interpreted by the team.' Here the two independentclauses – ‘The research was conducted by an in -depth survey’ and ‘the results were interpreted by the team’ –should be separated by a semi -colon (‘The research was conducted by an in -depth survey; the results wereinterpreted by the team'), a full -stop (‘The research was conducted by an in -depth survey. The results wereinterpreted by the team’), or a conjunction (‘The research was conducted by an in -depth survey, and theresults were interpreted by the team’). Colons: A colon is generally used to introduce more information. The information can take the form of a list, phrases or clauses, or a quote. For example: Please bring the follow ing items to class: your homework diary, anotebook, your pencil case and your textbook. The instructions said: 'Connect part A to part B'. Comma usage: A comma should be used to separate interrupting words/phrases/clauses from the rest of thesentence. Thes e interrupting words/phrases/clauses should add extra information, but they should notchange the actual meaning of the sentence if removed. For example: That student, John, was very helpfultoday. Mrs Smith, the new sports teacher, showed us a new technique . John, who has just been made aStudent Councillor, is presenting his speech at assembly. 3 Healthcare ethics Healthcare ethics/Medical ethics can be known as the application of core principles of bioethics to the decisions made in a medical or a healthcare setting . The ethics help in providing service through multidisciplinary lens, to the public and to understan d their complex issues so that the recommendations can be made for complex issues (1) . These ethical codes and issues are applied on all the Healthcare providers (HCPs). Pharmacists are also the healthcare providers that help in providing the people with a ppropriate medication so that the patients can be provided with appropriate treatment (2) . The branch of pharmacy is also works under the ethical code of conduct. The paper describes the scenario of a pharmacist who has been reported for knowingly providi ng employees with ‘Absence from work certificates’ for gaining additional pay for them and will apply code of ethics and the breach of legislation regarding the case scenario. In addition, the paper will also assess the ethical problems that occurred durin g the practice. And will apply the consequences that can be faced by the healthcare professionals when breach of legislation is reported. The paper will provide Gibbs’s reflection on the scenario (Your tone is too informal here. Try to choose words that are more formal. These words can simply be replaced with an 'is' (This area is missing words or phrases that, if added, would help your writing be more grammatically complete and clear to the reader .), Ethical codes, legislation and standards Code of ethics can be considered as the codes that helps in supporting the pharmacists and forms a base that helps the pharmacists to apply the practice with responsibilities. These codes or principles are based on the moral obligations and the virtues which are establis hed for the pharmacists as a guide to the relationship between the pharmacists and the patients (6). The pharmacy board of Australia has developed various codes and guidelines according 4 to which a pharmacist should work . Along with this , the Australian hea lth Practitioner regulation agency has launched the guidelines and notifications for the registered healthcare professionals along with the help of the National Registration and Accreditation agency (4). The pharmaceutical society of Australia released th e code of ethics for pharmacists which helps in the articulation of the values of the profession of pharmacy and the expected standards of the ethical behavior of the healthcare professionals or pharmacists towards the individuals, public and community (5) (The meaning here is unclear. I'm not exactly sure what you are trying to say here ). The three codes of ethics released by PSA (PSA With acr onyms, spell out the words on first use, put the acronym in parentheses ( ), then use the acronym for all subsequent uses. are Care Competency, and Integrity The code of ‘Care ’ explains that a pharmacist makes the health and the well being of a particular individual of a patient, his first priority and practices and promotes patient centered care. Along with this, a pharmacist also exercises professional judgement in the interest of the wider community and the patients (6) (Your tone is too informal here. Try to choose words that are more formal. Consider a more formal term (e.g. additionally or moreover A colon is needed here in order to improve the clarity of the sentence. Please refer to notes in the Feedback Summar y). The code of ‘Competency’ explains that a pharmacist demonstrates commitment for the self -development and the de velopment of the pharmacy practice. A pharmacist also works in collaboration with the others so that they can deliver a person -centered care and can increase positive treatment outcome (3). The code of ‘Integrity’ explains that a pharmacist acts with integ rity and remains honest with the patients so that they can maintain trust and confidence with the patients. Along with this, the pharmacists only work in those situations which upholds the professional judgement (6) . 5 Along with this, the law of Health Prac titioner regulation national law (ACT) was also launched by the government of Australia for the effective working of the healthcare professionals in a healthcare setting. the objectives and guiding principles of the law is to regulate the health practition ers and the registration of the students who are undertaking the practice of healthcare practitioner along with the clinical training (7) . The objectives of the law are: To provide the public with the protection and with trained and qualified practice. To facilitate the workforce mobility of the public across the country so that the b burden can be removed from the healthcare practitioners (7) . To facilitate the high -quality practice to the public with the help of education and training regarding the heal thcare professionals To facilitate the responsive assessment of the trained HCPs (7). The standards were also released by the government which relates with the common aspect to all the allied healthcare professionals. There are five standards which provide guidelines to the healthcare professionals who work in ACT health regarding the professional practice (8) . (You have repeated this word. Please change them if they are not direct quotations. )Along with this, these standards help in informing the community about the required professional practice for the HCPs who work in ACT health (8) . Also, there were guidelines that were released for the pharmacists under the fair work act 2009, relating to the issuing of the absence from work certificates that explains the responsibilities of the pharmacists to issue the certificate (8) . The ethical problems relating to the case scenario The case scenario is based on the breach of the ethical conduct and the legislation of ACT in which a pharmacist was reported for knowingly providing the people with ‘Absence from 6 work certificates’ for gaining additi onal payments (9) . The pharmacist who provided the certificate was not in the area of expertise for providing the certificate to the patient. Moreover, the pharmacist did not ideally confer with the illness of the patient. In addition to this, the pharmaci st did not keep the copy of the patient’s condition and did not issue the patient with the fee certificate. Along with this, the patient was unaware that the fee, the pharmacist took was for his own benefit and the certificate was not authentic. The practi ce breached standards, ethical codes and guidelines were released by the Australian health practitioner regulation agency, pharmaceutical society of Australia, National Registration and Accreditation agency, etc. The provision of the certificate comes unde r the pharmacists who have obligations in providing the certificate and it is mandatory that they provide real and authentic certificates to the patients so that there are no future complications (6) . (The word choice is inappropriate for the context. Please try to rephrase this. I think what you are trying to say is that the individual was not qualifie d) The practice breached many rules, regulations, laws, s tandards, etc. Under the law of First work act, 2009 launched by APHRA , it has been established that a pharmacist is allowed to provide the people with medical and absence from work certificates but they should be authentic and verified and a record of the fee and the consultation should be maintained . This law was not maintained by the pharmacist as the pharmacist never kept the record of consultation and certification fee (10) . Along with this, the pharmacist is obliged to make a person satisfied by the t reatment and is under the obligation to provide the people with complete, truthful and accurate information about the services before the consultation (10). Th e legal obligations of the pharmacists are that they should supply, compound and dispense the medicines and provide the professional pharmacy services which includes advice relating to the minor conditions and the safe and effective use of medicines (10) . The guidelines launched by PSA establishes that a ph armacist should only provide the people with the 7 certificate when they think that the knowledge provided is considerable regarding the health issue of the person. In addition, it can be said that any pharmacist is not obliged to provide the people with the absence from work certificatio n upon the request. The pharmacist, in this case, never provided the patient with face -to-face consultation and did not submit any written request of the certification. These above -mentioned obligations were not followed by t he pharmacist as he stood unaware of patient’s situations and never made an effort to assess the condition of the patients which resulted in malpractice for personal gains (The focus of this paragraph becomes a little unclear. Remember to stick to one main idea per paragraph Please, ADD mentioning the STANDARDS of practice WHICH ARE BREACHED ACCORDING TO ‘AHPRA’ AND ‘PSA’at least 5 -7 standard in points) . A pharmacist is trusted by the patients as they are certified by the Australian Pharmacy Board so it is the duty of a pharmacist to provide the patients with truth ful and authentic documentation ( You have repeated this phr ase in close succession. You might want to consider just using 'their and Try and avoid the use of unnecessary words as it leads to wordiness and poor readability .') Working under these rules also helps the pharmacist to provide the patients with authentic and valuable certification and can stop the consequences that are faced by the people who present fake certificates in their workplace and prevents the cancellation of the pharmacist’s license (10 ). Other responsibilities of the pharmacists include that the people who ask for the certificates should be provided with a confidential and decisive environment so that they can openly express their problems and/or medical conditions. (6) . The pharmacist should ask appropriate questions that can help in the asse ssment of the condition of the patient . Do me a favor stop repeating useless words and sentences, it is not relevant in this case ) The pharmacist also neglected the code of ethics relating to the provision of the services to the patient as he did not take the patient’s health and well -being on his first priority which led 8 the patient to face problem in the workplace (6) . Moreover, the pharmacist did not provide the patient with the person -centered care as is mentioned in the code of ethics. He also did not provide the patient with the continual development of his practice as continual development of the practice as it aids in providing the patients with effective and quality healthcare supplies and improve the health outcomes of severely affected patients . The pharmacist did not maintain the trust and honesty and neglected collaboration with other healthcare professionals which would have helped him in gaining maintaining competence with the healthcare professionals (10). The standards established by APHRA and PSA were breached in this case. The breached obligations were that the considered it appropriate to provide the patient with certificate even without any face -to-face consultation and without gaining any information about the patient’s condition (9) . The pharmacist was not qualified to assess the patient’s condition. In addition, the pharmacist did not recommend the patient to the doctor for further consultation. The pharmacist did not provide the patient with the truth behind his working and with the certification of consultation fee (9) . The pharmacist never informed the patient about the price of the certificate. Along with this, the date, date of consultation, legibility of the certificate, etc. , were fake. The pharmacist never received the req uest for the certification from the patient and did not ask for it. Along with this, he never provided the patient with the background of his service. In addition, he never completed the record for consultation and made an assessment in relation to the inj ury or the illness of the patient (10) . Potential consequences for the patient The provision of fake ‘absence from work’ or medical certificate can impact the patient adversely and can result in the termination of the employee who provide d fake certificat e. Along with this, the provision of fake medical and absence from work certification can be 9 taken as a criminal offence ( 11) . In addition to this, wrong assessment of the patient leads to collection of wrong information which results in the making of unau thentic certificate including irrelevant injuries and issues. As a result, the certificate contains false information which makes the employee a criminal , as he has run against the policies of the company (12) . Consequences faced by HPC and the steps that can be done to avoid the practice in future (This heading is quite wordy. Please try to rephrase it and make it more concise .) Being a pharmacist, there are certain duties which should be fulfilled . I can say that pharmacists who provide the patients with fake medical certificates can be taken into custody for not abiding by the laws, l egislation and the standards that are made for the professional practice and can face legal actions and prosecution (13) . There are specific steps launched by the APHRA to avoid this malpractice in future (9) . The pharmacists of healthcare professionals ca n seek for a request certificate at the time when the people come seeking ‘absence from work certificate’. This step is followed by providing the person with the information about the service and the working of the pharmacist. After this, a consultation of the person seeking ‘absence from work certificate’ is conducted followed by the record keeping of the consultation. The pharmacist can then follow the step of assessing whether the certificate can be issued in the relation with the illness or the injury o f the person. The pharmacist then completes the certificate and receive the payment for the certificate and stores the record regarding the issue of the certificate in an appropriate manner (9 ). The pharmacist can face issues relating to the provision of the fake certificate as it degrades the respect and the autonomy of the patient in him workplace setting as mentioned in the Australian Capital territory Law (14) . The potential consequences can include the determination of the negligence and the imposition of medical malpractice lawsuit along with other actions like cancellation of pharmacist license (7). I think that the pharmacist can be 10 accused of not providing his practice according to the standards made by the ACT health which tells to be tru thful and honest with the patients and providing them with the rights of the patient along with therapeutic communication and professional boundaries (8) . Along with this, the practitioner of HCP can be accused of not providing the patient with evidence - ba sed practice , responsibilities of the HCPs and not working under the scope of practice which could have resulted in further consequences in the patient’s life (8). According to me, HCP can avoid these consequences and irresponsible practices in the future by working in accordance with the standards, laws, legislation, guidelines and the principles that are provided by the government of the country. Moreover, the pharmacists can work with discipline and values that can help in eliminating the ethical miscond uct (17) . Gibbs’s reflection Description : After looking upon the case, I can say that a pharmacist was reported for his unethical conduct and for providing the people with ‘absence from work’ certificates for gaining money .( Try usin g more clear and concise wording, to improve the clarity of this sentence. There are several better options: considering, reviewing, assessing, etc .) I consider this as a negligent situation as it can ruin life people because of the negligence, the healthcare professionals present towards the public . I assessed that the pharmacist was not fully aware of his obligations and as a result, he was reported by the people for his malpractic e. Feelings: I can say that the case discussed , support s unethical values and neglect shown regarding the health and feelings of the public. I can say that it is an unfortunate situation for both the practitioner and the patient and cause d them to suffer severe consequences. According to me, the practice of the pharmacist was very selfish and destructive , as he was a social service provider and was providing his service to gain money. 11 Evaluation: The cas e can be considered as full of neglect and selfishnes s. The situation is destructive as it can result in potential life long issues .( Try using more clear and concise wording, to improve the clarity of this sentence .) The HCP in the case was very self -oriented and showed negligence towards the patient. He expressed unprofessional and unethical behavior which can become a cause of both the patient and himself. He also faced a problem of being reported as a fraud. As a colleague pharmacist, I think that h e should have discussed and collaborated with me and as a result, I would have provided him with important rules and norms of pharmac y. Analysis: The situation was unethical and degrading for both the patient and the pharmacist. As a colleague, I can say that the situation could have been prevented if my colleague would have considered discussing his malpractice . I think that the steps provided by the APHRA and PSA can always be included in pharmacy as they provide clear information about the practice whic h can prevent misconduct. The unethical situation could have ruined lives of people which is why , I can say that the reporting of this situation was mandatory. I would say that unprofessional behavior is not needed and should be reported immediately so tha t the consequences of the malpractice can be prevented. Conclusion: As a colleague pharmacist, I would never give a fake certificate to the patient as it can degrade a person’s life and would have led me to severe consequences. Situations can go out of co ntrol and can lead consequences like the cancelation of pharmacist’s license which can ruin a HCP’s whole professional life. I would completely assess the patient and would provide him with an authentic certificate with complete explanation of the patient’s situation in my future practice. Action plan : As a colleague, I learned from the assignment that I would never keep a patient’s life at risk and would never breach the laws made by the government regarding the practice 12 of a pharmacist .( Try using more clear and concise wording, to improve the clarity of this sentence .) I will al ways work in accordance with the standards and guidelines made for the pharmacy practice. Along with this, I would look upon treating the people professionally and will communicate with my colleagues to work in a situation. I will work with the steps recom mend by APHRA and will never disappoint people through misconduct and malpractice. 13 References 1.
