You are caring for Mr Berry, a 92-year-old gent. He lives with his daughter, her husband and two children. Mr Berry has a diagnosis of dementia that effects his ability to attend his ADL’s completely independently. You work for the home visiting nursing service and will be visiting to assist with the caring role and ease the burden on the family. Mr. Berry can attend to his own hygiene and dress himself but requires prompting and supervision. He can take his medications but requires prompting as he often forgets. Mr. Berry and his family expressed their wish for him to maintain his independence but also ensure his safety. During the visit, the family request you recommend services or organisations for further assistance and support. They live in the same local community as you and your family.
