Using the template below:
1. Describe the situation you are faced with. Identify your role and responsibility in the situation. Discuss how the situation touches your vales, thoughts and emotions (concern)
2. Identify and discuss how you know the situation is affecting you (as evidenced by)
3. Create 2 SMART Goals, with clear, specific language that identifies your desired change. No more than two sentences each. (what do you want to be different)
Create 3 Strategies For Change (for each goal) that you will utilize in order to achieve your goal. Include a timeline for when you will utilize each strategy – for example “I will book an appointment with the Program Coordinator by October 3rd. (how are you going to do it? What’s the plan?
o Identify 2 Indicators (for each goal) that will allow you to know when your goal has been achieved. Include a timeline for when the goal will be achieved – for example “this goal will be completed by November (How do you know when you’re done?)
PART TWO
Using the updated goal sheet and a 10 minute audio recording, explore and address the following:
· Reflecting on the progress on your goals, complete Goal Sheet Part Two :
o Document the progress of each of your goals (Outcomes). There must be meaningful comment on your goals and all three Strategies for Change. Describe how you implemented your goals and the effect of your Strategies. How has your situation changed or remained the same? Use your evaluation criteria to comment on the outcome of your efforts. (6 marks)
· In your audio recording:
o Discuss whether you need to reassess your original definition of the problem or reassess/revise your goal (Revised Goal).
o Discuss how this problem has affected you. Discuss the impact on you and others involved in the issue and any dissonance you experienced. How do you feel about the problem and about the process of problem solving now?
o Identify your communication skills during this problem-solving process. Evaluate and label your communication style, and skills (productive and destructive) that were evident while you worked through this problem. Remember to consider non-verbal as well as verbal communication skills and be honest!
o Identify your strengths. (What did you do well, describe with examples.) Identify your challenges. (What challenged you the most? Why? What do you wish you could do better? Describe with examples.
