Set the scene for your essay: briefly identify the patient and the context of care. (Remember confidentiality issues and NMC requirements).  For example:

“The essay will focus on the care of Margaret (pseudonym), who was admitted with breathing difficulties to a medical ward .  The first part of the essay focuses on communication and the second part will discuss nursing care. I will conclude by reflecting on my development as a result of participating in patients care while on placement”

Part 1

Identify what the communication issues were in relation to the care of this patient.  For example, you might focus in our example above on the difficulties communicating with a hypoxic/confused patient and how this was handled.  Ensure that you relate this to theoretical perspectives, e.g. showing empathy, active listening skills etc.  How did the interprofessional team communicate (verbal and written) in relation to the care of this patient, could anything have been improved.  How were the relatives involved in terms of communication?

If you choose a surgical patient you might focus on pre-operative communication or for a medical patient being discharged home, communication with the community team might be the area to look at.

In this section we want to see that you are able to apply the theories you have learnt related to communication to your patient scenario.

Part 2

Identify the nursing care problems you will focus on and why: What is Roper, Logan and Tierney’s Activities of Living model, what problems did you choose and why?

What is the Nursing process?

Assessment- What general assessment did you undertake, what objective and subjective assessment did you do? Did you use any assessment tools? What did you find?

Goal- What short and or long term goals for your patient did you identify? Use SMART goals

Care and implementation- What care did you initiate? Why? How was this implemented?

E.g. To aid the patients breathing the patient was sat upright Briggs (2015) suggests that sitting a patient up aids their breathing by …….. In order to aid this the pillows were positioned …… as per Bloggs (2015)

Or Oxygen was administered via nasal specs as suggested by Briggs (2015) to aid comfort for the patient and allow them to continue drinking fluid. Oxygen was administered at 24 percent which was prescribed by the doctors in order to maintain the patients oxygen saturations to 95 percent as per British Oxygen guidelines (2012)

For your conclusions draw together the parts of your essay and highlight in a reflective manner how you have developed as a nurse because of being involved in the care of adult patients while on placement.  What have you learnt about communication strategies, communication within the interprofessional team, planning care using the nursing process etc?  Remember you are a first year student and you might identify areas that you will improve on as you progress in your nursing course.
