The healthcare system needs a collaborative and an interpersonal approach for making a plan care. The healthcare providers and the end users contribute to the successful healthcare plan and implementation of it. For the safety of the patients, transparent and effective communication is very important.  This offers the clinicians to formulate techniques to deal the patients and involve the patients in a decision-making process. Nurses contribute to the healthcare systems by documenting the track records of the patients that help to study the patients and the growth of the treatment. The importance of all the factors is discussed in this essay.

Importance of patient safety and communication for patient centered care

National Patient Safety Foundation 2008 defines patient safety is the protection to the recipients of the healthcare by the individuals of the organisations from any kind of harm (Levett, 2013). The safety includes physical, psychological, emotional & cultural safety. The safety of the patient refers to the healthcare systems that are trustworthy and minimise the incident and impact and brings recovery in the adverse events in maximum cases. In order to minimise the complexity and tensions, there has been needing of communication skills for the healthcare organisations (ACSQHC, 2012).

Communication is called as the two-way interaction in which the information, meanings & feelings are communicated, both verbally & non- verbally. The message that is conveyed is understood as per the expected outcomes. Communication in healthcare sector refers to the imparting clear and true information to the patients in the accurate time.  The activities of listening & talking also involve in the course of listening.  Due to inability to see the whole situation we make the erroneous conclusion (ACSQHC, 2015). 

The expectation of the patients is to get inclusive, appropriate & timely information.  The Australian Charter of Healthcare Rights focuses on the need of communication and impact of working with patients for safe care. The survey made in the year 2009 on the patients of Australia and their families revealed that priorities are only on the healthcare during they undergo it.  The survey states that effective communication is a basic need for the healthcare. The survey discussed that patients are confident on the activities of the healthcare professional and the professional discuss their anxieties and fear.  The doctors & the nurses answer the patients as per their needs and the patients are given adequate information. The test results are clearly communicated to the patients. The patients are also given chances in the decision making. The rights & responsibilities of the patients are communicated. The staff of the healthcare takes possible measures to control the pain of the patients. 

Effective communication and appropriate compliance reduce the stress, anxiety, manage pain, mood, self- esteem, develop psychological status, lessen the duration of staying in the hospital, reduce cost,   increase the satisfaction of the patients, focus on the safety of the patients and reduce errors.  Poor communication with poor compliance brings hostile and confusing situations. It will create the lack of trust and brings lots of risks and harms to the patients.

Patient-centered care is considered as one of the most important attributes of the patient-safe communication.  The current healthcare systems consider patients as the active members & part of the healthcare team (Duckett, 2009). In order to ensure the safe care patients & their families consider as a vested interest.

Patient-centered care focuses on the principles of showing empathy, respect, choice, clarity and eagerness to assist individuals to lead the life the way they want. The study states that the patients bring their own experiences, skills & knowledge about the conditions & illness that they have.  With a mutual partnership of the healthcare professionals & the patients & their families the planning, delivery & evaluation of healthcare has been done (Harrison, 2016).  The patient-centered care is applicable to all people and offers possibilities to practice it in every healthcare setting.


The people with ethical, open-minded, self-awareness & responsibility for actions can practise patient-centered care. They consider the patients as the paramount importance for them.  The clinicians of the patient-centered believe that the life pattern of the patients influences the experience of the healthcare of them. Thus they try to learn the perspective of the patients. They involve themselves in their care and also promote the involvement of the family and friends of the patients (ACSQH;, 2010). The clinicians also extract the preferences of the patients. The information is communicated to the patients and they are also involved in the decision making.  The culture, belief & values of the patients are respected by the clinicians. The patients are offered comforts & support both physically & emotionally. The dignities of the patients are maintained and the processes of the care are designed as per the need of the patients.  Thus the coordination is ensured transparent accessibility to the healthcare system is offered.  The clinicians are also very sensitive to dimensions of care that are non-medical and spiritual. The major area of concern for the clinicians is to impart education to the patients to protect themselves and their health from the further recurrence of a disease.   The clinicians offer support for self-care & management. The patients are communicated information regarding risk & probability of the risks (hccc, 2017).

Documentation in the healthcare sector is also very important.  The interaction between professionals of healthcare & the patients is recorded. This includes the assessment, identification of health problems, planning, implementation and evaluation of care.  For the management of the data and information of the patients, Management Information Systems is used.  This is treated as the legal record for the research, analysis and decision making. This offers accurate information for the use (Hall, 2016).

The role of nurses in documenting the care is important. The documentations are required for review and revise so that efficient, effective & cost-effective care will be provided with the patients or the clients.  The nurses require ensuring the improvement of quality in this process and all the documents are updated without any error.  This will help to record the progress of the patient and compile with the data that are related to health Thus process of documentation is very important for every organisation. 


Healthcare management has been the very important concern for the healthcare sector. Communication plays an important role in the management process of the healthcare system. The MIS also contributes to keeping the track records of the patient’s history.
