Case of Audrey Thomas:  Day 1

Audrey Thomas is a 66 year old female.  She lives on his own, since her husband died two years ago.  She has been overweight and a smoker for many years.  She is 165cm tall and weighs 110kg.  Audrey is constantly tired, and spends a lot of her time during the day on the couch in the lounge room.  She has laboured breathing and severe dyspnoea, her respiratory rate is 32 breaths per minutes, she coughs a lot and brings up mucus, and has a barrel chest.  Her heart rate is 120 beats per minute, oxygen saturation SaO2 is 91%, and she has been diagnosed with COPD.  

  1.  Which disease/s within COPD does Audrey most likely have?  For each disease/s that you consider, ensure that you use evidence from her case to support your diagnosis.  For each disease/s that you consider that Audrey has, explain how the pathophysiology links with her clinical features. 
  2.  Discuss which aspects from Audrey’s case demonstrate that she is attempting to compensate for her respiratory condition.  
  3. Discuss Audrey’s risk of suffering from respiratory infection (compare her risk with that of others in the population and explain why).  
  4. At this stage, Audrey does not have a pulmonary embolism.  Discuss any aspects in her case which may increase her risks of developing a pulmonary embolism, and how she could be managed to prevent this developing.  

Case of Audrey Thomas:  Day 2

Earlier today, Audrey had a fall and has fractured her hip.  This resulted in a full hip replacement, and she is now recovering from that procedure.  

To assist with your preparation, it is suggested that you access the following site which explains how to calculate and interpret the body mass index (BMI):  

  1. Discuss which muscular and skeletal conditions Audrey may already have, or is at risk of developing (consider before and after her fracture).  
  2. Discuss factors which may have contributed to her having a fracture. 
  3. Interpret the events from today in relation to her respiratory system, by considering how today’s hip fracture and replacement may influence respiratory function.  Include discussion of her existing respiratory condition/s.

Case of Audrey Thomas:  Day 3

Audrey remains bed-ridden as she recovers from yesterday’s hip surgery.  Her blood test results show the following:

  1.  Discuss the urinary volume that would be expected for Audrey, and why this volume would occur.  How should this be managed? 
  2.  Interpret Audrey’s blood test results including any diagnosis which may be relevant to her.  Include aspects about Audrey that may explain her blood test results.
  3. Reflect back on Audrey’s health from day 1 onwards, and discuss what complications Audrey is at risk of developing in consideration of her current health conditions.
