Case Study: North Shore Community Hospital (NSCH)

Background:

Government of Queensland has intended to carry out initial analysis to build a community hospital to meets the needs for suburban community. This new hospital, to be considered as a system that will be a part of a larger “Community Healthcare” capability in the northern part of the Brisbane City. Hospitals generate a wealth of data round the clock, 365 days a year, all of which needs to be well managed to ensure efficient functioning.

Important NSCH Aspects to Consider Requirements Management

The recent trend sees a growing emphasis on improving overall efficiency and clinical management. In the requirements gathering phase, one should undertake and perform a comprehensive impact analysis of all business processes that exist, the stakeholders involved, and the systems / applications currently being used within the establishment that would be affected, albeit to varying degrees. Once this has been finalised and agreed upon, a requirements analysis followed by documentation needs to be prepared.

Requirements

An Overview Generally, the system should be safe and secure from a data management point-of-view. The anticipated mission of the new hospital is to provide the variety of health services for the those in need. Highly sensitive data is handled by such systems and hence the comfort-level related to privacy and safety issues need to be addressed aggressively.

The system should ensure efficient flow of information that provides interdepartmental support to the establishment, functional and process integration, be adaptable and flexible from a user perspective, and last, but not the least, be standards-based to ensure interoperability in terms of syntactic, semantic and process.

The following points are that need to be given serious attention in order to build and implement a viable solution that will be able to deliver true value-for-money on a long-term basis:

  1. Each patients’ details will be entered into the system once they arrive for treatments. Use of a unique patient identifier like UHID (unique health identifier); There could be quick registrations in times of emergencies as well as wait listings for surgeries.
  2. Data security, patient confidentiality and privacy; All the users and employees who work in the hospital must identify themselves using a password, and biometrics and roles in the system are assigned to them when the first employed. Only users who are authorized in this could access the systems data.
  3. Eligibility check of all insurance and ability to accept an upfront deposit to cover the estimated cost of care;
  4. E-prescription for outpatients, Computerized Physician Order Entry (CPOE) for others;
  5. For investigations, the consultant needs to know the total costs and the individual investigation charges. This would allow them to prioritise the ones that the patients must get done right away irrespective of the costs while leaving the rest later when they can afford them.
  6. The Electronic Medical Record (EMR) needs to be integrated with Laboratory Information System (LIS), Radiology Information System (RIS) and Picture Archiving and Communication System (PACS) to allow all images to be viewed and compared with any archived images;
  7. Secured remote access to view information and add notes;
  8. Checking for Economic Order Quantity (EOQ) and re-order levels and automated listing of near-expiry items at least 90 days prior to expiry;
  9. Slow moving materials in the medical stores should be tracked and appropriate alerts should ensure that all stakeholders are aware of the situation;
  10. Bar coding for tracking patients, services, material and medication; and
  11. Management Information System (MIS) reports that serve as de facto registers will need to be maintained as per prevailing rules, regulations and legal requirements

Assignment Requirements:

You are required to write a business report for the NSCH management team which is formatted in a professional and meaningful structure. Please note, the standard report structure must be adhered to as per ‘The Communication Skills Handbook’ (Summers & Smith 2014).

You are permitted to make reasonable assumptions where necessary, but these should be documented.

Review the SCH case study and answer the following questions with reference to the information in the case study.

Do NOT extend the scope for any of the following solutions beyond that specifically described in the case study above.

It is recommended that you review all documentation for this case study before finalising any single solution. Ensure that the required consistency has been included within and between each question solution. You may use software such as Microsoft Visio or www.draw.io to draw the models; neat hand-drawn models are also acceptable.

  1. Task 1: Approaches to Systems Development

There are many approaches to Systems development such as Water fall SDLC, Agile, RAD JAD. etc. You need to clearly explain which approach is suitable for your project and why. Give justifications of choosing your approach. Justifications can be provided by discussing the advantages and disadvantages of approaches.

  1. Task 2: System Information Requirement Investigation Techniques
  • Who are the stakeholders involved?  

Stakeholders are those people who have direct and indirect impact and have an interest in the functional and non-functional requirements of the system being developed. They can range from technical to non-technical people and from internal to external people. For the NSCH project, who are the internal and external stakeholders that have interest and impact to the project?

  • Information Gathering Requirements

Gathering detailed information and defining requirements for a new system are important systems analysis activities. For this project, the NSCH management team are considering hiring a consultant to undertake the requirements gathering for their new system. The NSCH management team want you to write a report explaining these activities and why they are important. Explain how you would undertake these techniques, what information you would hope to acquire and why these techniques would be effective. In your discussion, you must apply your knowledge to the case study and give examples that will enable the NSCH management team to better understand the nature of information gathering.

You need to clearly address and explain all Functional requirements. Why you would like to have your stated non-functional requirements.

Non-functional requirements are the description of the restrictions and constraints that the system must meet by providing its overall qualities and attributes. These qualities and attributes describe how the NSH must be developed as stated in the case study.

• What are the non-functional requirements for the system in the case study? List and discuss these non-functional requirements. Justify the choice of your non-functional requirements.

  1. Task 4: Find out all Critical Use Cases for the HIS you are developing and Use Cases diagram  

Use case is a list of actions or event steps typically defining the interactions between a role (known in the Unified Modeling Language (UML) as an actor) and a system to achieve a goal. The actor can be a human or other external system.

  1. Task 5: Domain Modeling and Class Diagram

Part A: Domain Modelling

Review the case study to prepare a domain model class diagram for the supporting information system.

Solutions must follow the methodology as outlined in the prescribed textbook (Satzinger, Jackson & Burd 2016). Solutions are expected to show:

  • The class name and attributes list for each class and sub-class as required
  • All required associations
  • All attributes as specifically mentioned in the case study must be reflected
  • Other attributes as needed to support the described functionality.

It is not necessary to show methods; however, you may include them if you wish. Solutions are expected to align with the components as shown in figure 4-16 and 4-21.

Part B: Design Class Diagram

Prepare a Design class diagram for the ‘Patient and ‘Doctor’ classes ONLY. These two classes should be part of the Domain model class diagram solution for the previous question.

Each of these design class diagrams are expected to have a complete attributes list and a comprehensive methods list which supports the specified functionality as described in the case study.
