Question One: Methotrexate is used in the treatment of a number of auto-immune conditions, including rheumatoid arthritis. With a focus on the underlying pharmacology, provide an overview of at least two pharmacogenomic markers that have been associated with response to methotrexate when it is used for the treatment of rheumatoid arthritis. 

Question Two: Anthracycline antibiotics are very commonly used chemotherapeutic agents, and one of their most serious side effects is cardiac toxicity. Describe the likely mechanism, any risk factors and an overview of how this toxicity may be prevented, focusing on the underlying pathophysiology and pharmacology. 

Question Three: Many agents used in the management of Multiple Sclerosis increase the risk of Progressive Multifocal Leukoencephalopathy (PML). Provide an overview of PML and describe the mechanism(s) by which the risk of PML can be increased with medicines used for the management of Multiple Sclerosis.

Question Four: A promising new treatment for inflammatory bowel disease is the use of faecal microbiota transplants. Provide an overview of how this treatment is effective in managing this condition, and the role of immunosuppressive medications in patients who have had the procedure. 

Question Five: A number of medications used to treat multiple myeloma are associated with the development of peripheral neuropathy (e.g. vinca alkaloids, thalidomide and bortezomib). Describe the mechanism whereby each of these agents are thought to lead to peripheral neuropathy and any differences in the characteristics of the peripheral neuropathy that is typically caused by each of these agents. 

Question Six: Over the past 20 years thalidomide and its analogues has made a resurgence in the treatment of myeloma. Provide an overview of the mechanism of action of thalidomide. With a focus on the underlying pharmacology, describe at least one advantage that newer analogues such as lenalidomide and/or pomalidomide have over thalidomide. 

Question Seven: Individuals that are allergic to honey bees can receive desensitisation therapy to prevent reactions if they are subsequently stung. Provide an overview of the mechanisms that occur during successful desensitisation, including the changes in the immune system that occur.
