Assignment Guidelines: 1. In groups1 of 4-6, or working alone, select a community of your choosing. (if working in a group, it is highly advisable that you develop a group contract to identify roles and expectations of group members at the very beginning). 2. Conduct a community consultation using relevant elements from the community assessment process found in Chapter 13 in the textbook. Given you will be in a virtual setting for clinical, you will not be able to speak to community members. Therefore, this dimension of data collection will have to be hypothetical. You may use a template or guide of your choosing to help you gather and organize these data (there are many available online). The following guide may be helpful in conducting the community consultation 3. Based on your community consultation and other relevant data you have gathered about the selected community, identify the relevant social determinants of health that emerge as a priority for this community. You should identify at least one or two (given that most of them overlap) that are under threat or are compromised for your selected community. Engage in a structural analysis of why the community is experiencing the problem(s) you have discovered [See Stamler, Yiu, Dosani, Etowa, & van Daalen-Smith (2020), Chapter 34, for some ideas of what is meant by structural analysis, as this is a very important step]. 4. Based on your findings from step #3, identify the community’s priority needs. Devise a goal for each need (if more than one) and create strategies to help meet the community’s needs (Remember: Given you completed a structural analysis; you will then respond to these needs by addressing structural issues. These might be long term, short term, or both). For each need, identify how it emerges from the community consultation and assessment process and describe the importance of addressing each need. Identify all resources to be used and accessed in order to facilitate health and address these needs on all relevant levels (local, provincial, federal, global). Engage with concepts of primary health care discussed in the course and demonstrate evidence of critical thinking using critical theory. It is also helpful to make use of (or parts of) a program implementation framework [See Stamler, Yiu, Dosani, Etowa, & van Daalen-Smith (2020), Chapter 14]. 5. List at least 2 actions per goal and describe how these actions lead to the achievement of each of the goals. 6. Create an evaluation plan for the implemented actions and associated goals. 7. Write up the project using 7th edition APA guidelines. Page limit: 20 pages2 including title page and references. If appendices are included, these are not to be counted as part of the 20-page limit and they can be in addition to 20 pages. Please note that eClass has a 100 MB limit on file submissions. Files that are larger than 100 MB must first be compressed before they are submitted. 8. Use a minimum of 10 sources, 5 of which must be scholarly. At least 4 sources must be from readings outside the required readings for the course. Please note that statistical reports and websites are not considered scholarly resources, and while they can be included in your reference page, they are not to be counted toward the 10 minimum references.
