The following are some tips for the assignment The purpose of this assignment is to provide you with an opportunity to develop a health promotion program and relate strategies of community planning, implementation and evaluation to the community diagnosis identified in Assignment 3, while facilitating your partial achievement of the course learning outcomes. Please note you are not expected to carry out the health promotion program that you develop.
The following are some tips for the assignment
1. Start with- intro stating the purpose and strategy
2. Give a brief description of the community as client/population in 1-2 sentences and specify the one priority community diagnosis from assessment assignment that will be used to help plan and develop the program.
3. Develop an outline for a health promotion program using the program logic model to meet the identified community diagnosis (include goals and objectives). Provide the rationale for the identified need, goals, and objectives for the program. Including a diagram that represents the program plan in an appendix would make the components of the program clear. Include the human, financial, organizational, and community resources a program have currently available that could be directed towards the program implementation. Include in the plan any resources and constraints. See the corresponding Moodle unit as well as Chapter 18 in the Canadian Community as Partner 4th edition text. a. When discussing goals and objectives for the program, remember the SMART acronym (Specific, Measurable, Acceptable, and Realistic to achieve, and have a Time line) that provides a succinct format and clearly outlines the parameters for the planning implementation and evaluation process. An Overview of Strategic Planning or "VMOSA" (Vision, Mission, Objectives that include SMART, Strategies, and Action Plans)
4. Compare and contrast the strengths and limitations of two health promotion approaches to influence change and promote health within a community, which need to be considered in the assessing, planning, implementation, and evaluation phases of a health promotion program. Approaches are strategies to use when one develops a health promotion plan and relates to the structure of the program. Choose 2 of the 5 approaches (community social action, community development, social planning, community organization, & Service Extension) listed in the community as Partner 4th edition text on page 262. There is also an exercise related to these approaches in Moodle unit 4 activity Define the 2 approaches in the discussion and support with literature. This section in the marking criteria is after how the influence of one approach would impact the assessing, planning that depends on outcomes desired for the program, implementation, and evaluation phases for the proposed program. Discuss in terms of how the processes and outcomes would have been described, measured, etc. using the different approaches. Include how each approach would influence the choices made within the program.
5. Include the role of the CHN in the approaches. Including the aggregate in the validation of the community diagnosis as well as when designing the program is important to include and support with literature.
6. Evaluation considerations and indicators are also definitely an important part in program planning for this assignment. For the evaluation state how you would evaluate the program and support with literature.
7. Ensure all points are addressed in the marking criteria and the assignment is within the 8 page limit excluding the title and reference page and the appendix in the count.
