Assignment 3: Community Assessment and Diagnosis It is recommended that you work through Section I, Units 1-4, and Section II, Unit 5 and Unit 6 prior to completing work on this assignment. The purpose of this assignment is to provide you with an opportunity to conduct a partial community assessment on a population and to diagnose areas of need for health promotion related to an identified population. This will be accomplished by collecting data using 3 segments from the Community as Partner model or 3 determinants from the Population Health Promotion Model depending on what Model you have chosen for this assessment and analyze the data that results in a community nursing diagnosis. The assignment will be easier when you have easy access or have experience working with this population. According to Vollman, Anderson& McFarlane (2017p. 222,), there are several ways to perform a community assessment and the first step is to determine the scope of your assessment. First, define the scope of the assessment: Geographic community? Or specific aggregate? Or Evidence of an issue that requires further investigation? For this assignment requirement, the scope was already determined in the marking criteria. In the assessment section for Assignment 3, the purpose of this assignment is to provide an opportunity to conduct a partial community assessment, and to diagnose areas of need for health promotion related to an identified aggregate/population/group. As you can see the scope is to be an identified aggregate/population/group for this assignment. Tip- Notice if you chose the population/group – the teen age population diagnosed with one or more STI living in specify the community; this would bring a different perspective to the data collected and be a different assessment than for – the teen age population living in a specific community. Also, when you select an aggregate with the identified parameter identified with them in their description, focus on their commonalities when using the 3 sections or determinants of health depending on the model chosen for the assessment. Also, when you select an aggregate with the identified parameter identified with them in their description, then it cannot be part of the issue of the community diagnosis as this is what identifies them. For example- the teen age population diagnosed with one or more STI. Instead, it would be the target population or focus of the community diagnosis. See point 5 for more information on the community diagnosis. Note that you are not required to interview or go out in the community to do this assignment; however, data is required for this partial assessment especially with the population/group description and the 3 determinants or 3 segments chosen from your selected model. This is how it is a partial assessment as you will notice there are more than 3 segments and determinants for each model. Data can be taken from literature, Statistics Canada, Health Canada and even from the local papers or news reports, if it is related to the population or key informants’ perspectives on strengths and issues affecting the population/group. When comparing different populations in relation to health problems or conditions; the rates for the community could also be compared with rates from similar communities, from the province or territory, or from Canada (Vollman, Anderson, & McFarlane, 2017). Caution, when assessing the population/group that bias does not occur from predicting the community diagnosis or program solutions before assessing the population/group. You may have some ideas what the issues could be in advance; however, it is important to gather information from the population/group, from key informants and also through Canada or city census and/through credible literature sources. Determine what the issues or stressors are as you assess the chosen population/group. What factors lead to a good health status? What issues does this population/group see as being the most important to them? Using literature and statistics to support your findings is also an essential part of this process. This provides you with broad knowledge of the population/group. The community diagnosis needs to be developed as a result of the assessment and analysis of the data. Finally, formulate the community diagnosis that includes the 4 parts as presented in Canadian Community as Partner Text 4th ed.. Page 256-257. This population and diagnosis will be used for final assignment 4. 1. To start the paper, provide an introduction that states the purpose and strategy of the paper. State the objectives of the paper – what will be included in this paper? These topics to be included can become your subheadings. Ensure they correspond with the criteria of the paper. 2. Clearly identify and provide rationale for the selected population/group (include demographic and epidemiologic data, as appropriate) for the partial community assessment. When discussing the population/group- -include the geographic boundaries that could be geographical or a common interest i.e. -aboriginal youth between the ages of ___ in district or community X. For example the homeless population in downtown Calgary specifies an area boundary. Include social demographics ie age, sex, ethnicity, culture, education, employment levels, socioeconomic status and the population/group's i.e. history, values and beliefs (use statistics and other references to support data). Include epidemiologic data, as appropriate for the population/group. Ensure you have all these points covered on the population/group as you will have greater success with this assignment. 3. Discuss assessment methods used or assessment methods to be used by identifying one of the models for choice, the Community as partner model or Population Health Promotion Model according to the marking criteria: provide the rationale for your choice. Depending on the model chosen, select 3 determinants of health, or select 3 segments from the community assessment wheel, and complete an assessment in relation to the population/group. 4. Analyze the information gathered – This should include the phases of community analysis (Classification, Summarization, Interpretation and Validation) outlined in the Canadian Community as Partner text. See (p. 244-chpter 17 4th ed) . When you collected the data on the population/group and used headings for the 3 determinants or segments depending on the model chosen, you were already classifying the information under categories. Analysis is summarizing and including your inferences on the data collected on the population/group using the chosen model. Clearly identify gaps in the data. Data gaps for the assessment means to identify the information you are missing that would be helpful for the community diagnosis. 5. Identify and provide rationale for the community diagnosis- this should be supported by the analysis of the assessment data for the population/group. One overall community diagnosis based on the assessment data collected for this population/group is required. Define if it is a wellness or deficit diagnosis. For examples, see the Canadian Community as Partner 4th edition by Vollman, Anderson & McFarlane (page 256-258) . 6. Use APA (6th edition) ensuring all the points on the marking criteria are addressed for the assignment that includes the page limit of 8 pages (excluding title and reference pages). Present and complete an analysis of the assessment data of the group using the chosen community assessment model in narrative form, point form, or as appendices. Ensure concise analysis of the assessment data and clearly identify gaps in the data. Identify and provide rationale for the community diagnosis (define if it is a wellness or deficit diagnosis) supported by the analysis of the assessment data for the group. Marking Criteria Each element of the assignment guidelines is addressed. Ability to analyze, evaluate, create, and engage in critical inquiry is evident throughout.
