Community Assignment

For this assignment, students will use critical thinking to assess a community and analyze the data to identify a priority community health concern. Students will then apply current literature to explore the implications, interventions, and community supports related to the identified community health concern.

Assignment Guidelines:

Part A – Complete the template answering the questions and filling in the charts.

1. Identify a community of your choice (ex. Whitby, Oshawa, Bowmanville) and outline its boundaries. Provide a map of the community in Appendix A. You may choose a subset of a community (ex. South Oshawa), but you must indicate this.

2. Do a windshield survey assessment of the community by completing the windshield chart in Part A. Provide three detailed and relevant assessment points in sentence-format for each area. This may require you to go out and observe your chosen community safely by foot or by vehicle* and it will require you to research information online.

3. Based on your assessment, identify ONE priority community health concern for your community. Cite three recent community reports or databases within the last 5 years that support this concern.

Part B

4. Write a scholarly paper describing the implications of the health concern on the community, the specific supports provided in the community, and the role of the community health nurse in supporting this issue in the community. See textbook chapter 9. Describe one CHN Standard and how this is demonstrated in regards to the topic of interest.

5. Provide a LIST of support services in the chosen community, relevant to the topic of interest. This list must include at least 3 support services, with name of agency, address, hours of operation, and a 2-3 sentence description of the service. Include this list as Appendix B.
