Adult Health Conditions and Healthcare Policy: Managing Safe and Effective Care

What am I required to do in this assignment?
You will be required to create a digital narrative of a person’s healthcare journey in which you demonstrate the planning, delivery and co-ordination of their care. The narrative must include detailed discussion of how relevant policies and legislation influenced the care provided and an evaluation of your own contribution to the person’s care.

Information has been passed down from generation to generation, through stories (narratives).  Your narrative will be presented digitally using a choice of technology that you feel comfortable with.  For example, you may choose to submit your narrative as a video or audio recording. You must support your work with a reference list/bibliography as appropriate.  

 

What do I need to do to pass? (Threshold Expectations from UIF)

Contextualise the case and setting of the healthcare journey, identifying an appropriate person from practice – maintaining confidentiality at all times
Describe the case utilising an evidence-based explanation of the person’s condition, their associated health need and the impact it had on them on their family 
Justify and clinically reason the care you planned, how it was coordinated and delivered, referring to any nursing theories, processes or models
Reference and discuss relevant policies and legislation referred to and detailed in the account
Appraise, your own role and your influence on care
