Development of a Community Health Promotion Program

Target population and rationale of health promotion

Introduction

A. It is found that the individual who are homeless are most likely found to be suffering from many issues most notably physical health issue and mental illness. They face social isolation and this disconnection triggers the development of higher rate of suicide and substance abuse disorder. The primary reason which resulted in the development of higher percentage of homelessness are poverty, shortage of affordable housing, unemployment, family violence, sexual assault, social isolation and addiction.

B. The issue of domestic violence is one of the sole factors which triggered the issue of homelessness in Canada. It has been found that the young Canadian aged between 12 to 24 years makes a quarter of country’s homeless population. It is highly essential to implement proper health promotion strategies in order to lower the percentage of homelessness population in Canada (To et al., 2016). The below paper will discuss about health promotion strategies in such as providing education, providing affordable house and increasing food security for reducing the issue of homelessness.

Discussion

1. The health promotion plan will be implemented for younger community people between the ages of 10-20 years as it is the adolescent were mostly found to be suffering from homelessness issue. The homelessness condition triggers the development of mental illness and increases the ratee of suicide among the teenagers especially among the Aboriginal population. The census reported stated that there is almost 26,743 Indigenous people are found to be experiencing the issue of homelessness and in the year between 2012-2012, in the specialist homelessness service centre almost 1 in 5 clients were found to be Indigenous client (Strobel et al., 2021).

2. Similarly, in the year 2016, around 19,400 children between the ages of 0-14 years were found to be homelessness and one of the issues which will found behind this fact was family or domestic violence. They young adults were found to spend night in streets, sleeping in temporary accommodation and supported accommodation for homeless. Another reason behind this issue is poverty which hampers the quality of life and increases the chance of experiencing homelessness by the children. Kozloff et al., (2016) performed a study for determining the impact of proper housing for homeless youth suffering from mental illness. The study determined that housing first concept was found to improve the housing stability among homeless individuals suffering from mental illness.

3. The study of the study showed that housing first concept helped in reducing mental illness among young adults along with that it lowers the issue of homelessness. Similarly, Flack & Razza (2021) stated that homeless children and adolescent are often found to be at highest risk of experiencing suicide in comparison to other homeless adults. The risk factors which were identified were sexual orientation, mental health disorder, victimization and duration of homelessness. The study highlighted that positive coping up strategies must be provided which will help in lowering the suicidality among the homeless children.

Health promotion strategies

The issue identified through the community assessment and which need proper attention or intervention for reducing the same are- delivering the homeless children with affordable home services, providing them food and providing free education in form of camps every week which will help in lowering the issue of poverty and increase the rate of employment (Wang et al., 2019).

1) the aim of this health promotion program is to provide the homeless children with proper safe home and lowering the issue of homelessness

2) To provide the homeless people with counselling and behavioural therapy which will help in reducing the issue mental distress.

3) To provide the children with peer support which will help in lowering the issue of suicide among the homeless population.

4) To provide the children with proper affordable home for reducing the issue of suicide and mental illness.

1. Jackson et al., (2020) performed a study for understanding the concept of tiny homes as an affordable housing strategy in order to reduce the issue of homelessness. The study stated that the advent of housing intervention helps in reducing the issue of homelessness.  The study determined that homeless youth are at a higher risk of developing mental and physical problems which results in the development addictive behaviour. The study determined that implementation of tiny homes as an affordable shelter for the homeless children helps in reducing the issues faced by the children such as staying in overcrowded area, or sleeping in street and their fry also helps in in reducing the chance of developing mental illnesses.

2. The implementation of these house within the community will help the adolescence in overcoming the barriers of gaining a suitable home and thus it will help in developing infection and risk of ginning mental illnesses.  Hence the choice of, affordable care houses implementation for the aboriginal community people is effective and appropriate. D’Sa et al., (2020) performed the study for understanding the psychological impact of childhood homelessness among the individual.  The study stated that children face is homelessness due to many issues such as lack of proper affordable housing, unemployment status and shortage of rental properties which results in devastating impact upon the poor community people.

3. The study determined that children who faced the homelessness issues are found to show delay in development and learning, increased level of depression and anxiety and lastly behavioural difficulties which results in excessive aggressive behaviour.  Hence, in this case the study suggested that providing the children with proper behaviour intervention such as cognitive behavioural therapy within the health care setting for affordable care houses will help in reducing the aggressive behaviour and it will help the children to have a proper academic learning by reducing the chance of trauma.

Providing affordable safe home service for the homelessness people

4. This intervention will also help in reducing the higher level of suicide among the adolescence suffered from homelessness.  The health promotion plan of implementing behavioural counselling therapy within the affordable care services is appropriate in this case. Haskett et al., (2017) performed the study for determining the impact of feasibility and acceptability of peer’s support group intervention in order to prevent child maltreatment among the parents experiencing homelessness.

5. The study determined that children and parents experiencing an episode of homelessness often faces a variety of detrimental factors which increases the risk of developing child maltreatment.  In this case parents support program was found to be one of the effective interventions for lowering the risk of child maltreatment. The study helped in understanding the fact that providing the parent with peer support helps in improving the relationship between the parents and the children and thereby lowers the risk of maltreatment which is common among homelessness individual.  Hence, in this case delivering the individual with their support will help in strengthening the relationship among the family and thereby reduce the chance of developing mental illnesses.

Providing food and education in form of camps within the community

1) the children will be provided with meal on wheel service which will help in reducing the issue of food insecurity

2) The children will be provided with education in form of camps which will help in reducing the issue of lack of proper education

3) Free health assessment and skill training will be provided for lowering the issue of lack of accessibility and discrimination within healthcare organisation and skill training will help in reducing the issue of unemployment

1. Loopstra (2018) stated that household food insecurity is one of the major issues in high income country and it requires a proper attention due to its detrimental effect upon the health of the homeless individuals. The study stated that implementation of proper nutritional and assessment program like community level intervention of food programs for food bank including meals on wheels services provided by the volunteers will help in reducing the issue of food insecurity among the homelessness individual.  The study also stated that food banks such as community kitchen or community shops needs to be active in providing them with proper food and diet which will help in lowering the issue of food insecurity and malnutrition among the vulnerable population.

2. Hence, the implementation of meals on wheels services will help in addressing to the need of food insecurity faced by the homeless adolescence. Woodgate et al., (2017) stated that youth transitioning is one of the effective phases which needs proper protection and care which will help them in becoming a self-sufficient individual.  The study highlighted that providing the homeless individual on lower income family children with proper education and housing will help in improving the transitional period to the adulthood by reducing the issue of poverty as educational intervention will help in increasing the chance of employability and thereby lowering the issue of homelessness and lack of proper education.

Rationale

3. Thus, health promotion plan for implementing educational program in form of camps for the homelessness individual is effective in this case. Malla et al., (2015) stated that it is highly essential to provide the young adults suffering from mental illness with the stigma free service which not only help in lowering the psycho-social issue but will also reduce the chance of developing advanced mental health disorders among the young individual. Thus, providing the homelessness children with free accessibility to health care services will reduce the issue of discrimination faced by the individual within the health care setting and it will also lower the chance of developing detrimental diseases among homeless individual.

A. The planning of both the health promotion approach such as providing the homeless children with proper tinny affordable house, food, counselling, education and services will be done by assessing the condition of the homeless children. A proper collaboration needs to be done in this case comprising of government agencies, immigrant services and non-government agencies. The young adults will be involved in the health promotion plan by providing them knowledge about the importance of the programs which will increase the participants. The knowledge about the services will be provided in immigrant camps and also in the parks or streets for involving more homeless children the health promotion plan (Elwell-Sutton et al., 2017).

B. The implementation of housing services will be done after collaborating with the government funding policies. The tiny affordable house will be provided to those individuals who are homeless where proper safe environment will be delivered for reducing the issue of trauma. The counselling sessions will be provided once in a week in the tinny affordable care along with that in the camps for providing support. All the children will be provided education based upon their learning abilities in the camps or in local community centres which will help in reducing the issue of lack of education (Gupton, 2017). The meals on wheel services or the food bank facilities will be provided to the individuals after issuing cards. The health assessment services will be provided to each of the individuals in the community centres.

C. The projects need a collaboration of all the professionals such as community nurses for providing free healthcare services, nutritionist for delivering free nutritional food, social workers for involving more children and behavioural therapist for providing counselling and mental support.

D. The evaluation of the health promotion project will be done with the help of questionnaire and survey which will help in determining the homeless individual’s perception and attitude towards the program. After 6 month of the project the participants mental illness will be assessed which will help in providing further intervention (Sharma, 2021).

Providing food and education in form of camps within the community

A. The community services in this case will provide treatment to the individual suffering from mental illness.  The nurses in this case will involve the patient in various healthcare programs within the community such as educational program, delivering peer support and also providing proper immunization to this adolescence which will help in reducing the chance of developing infectious disease.

B. Nurses will be responsible for performing a proper surveillance in the community in order to determine the number of homelessness children present prior implementation of the program and after implementation of the program which will help in understanding the effectiveness of the project (Goeman et al., 2019).  The nurses will provide proper education to the individual which will help in enhancing the skill and the knowledge and thereby reducing the issue of unemployment.

C. The nurses will also provide health education which is required for maintaining a proper health and well-being.  The health literacy will be provided in form of community discussion by involving the children and thereby understanding their health need. The nurses can also help the adolescence to develop a proper healthy relationship with the family which will help in lowering that the stress among the individual by providing family care intervention.  Hence community nurses play an essential role in providing proper service to the individual suffering from homelessness issue (Jego et al., 2018).

D. Weber et al., (2019) performed the study for determining nurse-led intervention for providing care to the population experiencing homelessness.  The study stated that homelessness is highly associated with lack of proper nutrition which results in poor health status and thereby effects both men, women and children suffering from this condition. The study also determined that almost half a million of people suffering from this condition are considered to be vulnerable group of population and requires effective care for lowering the chance of developing detrimental condition among this population and providing and equal access to this individual for add having same services such as public hospitals and shelters for reducing distress.  The nurses play an essential role in this case as they are involved in directly providing care to these populations by delivering them evidence-based practices and enrolling them in various camps for clinics and reducing the chance of developing deteriorating condition.  The nurses are found to have all the essential and necessary skills which will help the homeless population in assessing a proper positive health-related changes and thereby it will also help in improving the quality of life.

E. The nurses are also responsible for delivering holistic care to this individual and improving their mental, physical and social health issues.  The community nurses can also provide the homelessness individual with effective intervention such as providing free food services and enrolling in community camps which will help in reducing the chance of developing many diseases and reduce the issue of discrimination which is faced by this population. Stafford & Wood (2017) stated that homelessness is related with enormous health disparities, comprising of shorter life expectation, higher illness and superior practice of acute hospital services.

F. The nurses in this case can consider the individual’s social and economic factor as it the main reason behind homelessness and provide the individual’s with proper home, care and food which can be done by performing survey and community assessment of homelessness condition.

Conclusion

The above paper discussed about community health intervention which can be provided to the adolescents suffering from homelessness. It is found that the individual who are homeless are most likely found to be suffering from many issues most notably physical health issue and mental illness. They face social isolation and this disconnection triggers the development of higher rate of suicide and substance abuse disorder. Lastly, the paper discussed about health promotion strategies which needs to be implemented in order to provide proper support to this population.
