HMSV 3501 Case Management

Case Study:

May is a 47-year-old Aboriginal single mother of 3 children who are 12, 17 and 25 years old. May suffered a traumatic brain injury as a result of a motor vehicle accident. She was not the at fault driver in the accident. May required extended hospital admission due to the severity of her injuries. She also has a pre-existing mental illness (anxiety disorder). Due to physical impairments from her accident, she is now wheelchair-bound, and the health care team has determined that she is not able to return to her home due to accessibility issues. She lives in a 2- storey house with so many stairs. May’s current medical condition has exacerbated her anxiety symptoms, impacting her ability to participate in rehabilitation with her occupational therapist, physical therapist, recreation and speech-language therapists.

In the team’s meeting, the therapists are reporting to the doctors that May does not show any initiation/motivation to participate in therapy sessions. They are now considering discharging her soon to a care facility. In speaking with May as her case manager, she expressed the desire to return to her home and requested more rehab time to get her ready for community living. She shared her frustrations with a lack of empathy and understanding from the health care team. She identified her goals wanting to get better, so she can return to work and her normal life with her children.

During this meeting with May, you found out that May’s 12-year old daughter has been apprehended by children’s Services due to inadequate care from her siblings who were looking after her while May was in the hospital. Her 17-year-old son has not been regular in school and when he was in school, he was getting into fights and in trouble with the school authority. May is disturbed by these events with her son because this is his final year in high school. May also disclosed to you that she was in arrears in her rent, utilities, car loan payment, insurance companies and she is getting calls from these people she owes money while she is in hospital.

Utilizing the case study above or the case study from Assignment 3 (the selected movie), you will complete the following components of the case management process:

Collaboration:

The case manager collaborates around the strengths, needs and goals of the person they are supporting to ensure that the empowerment plan is “relevant, timely and responsive to the person’s [client’s] situation and needs” (Gursansky et al., 2012, p.16).

· Describe how you will inform all team members of ongoing updates for the person you are supporting

· Who else that is not part of the multidisciplinary team, would you need to consult/collaborate with and why? (Gursansky et al., 2012, p. 111).

Integrative Case management

Case management is an “integrated services delivery,” highlighting the importance of “blending of services for seamless service delivery” (Gursansky et al., 2012, p.16).

· Discuss based on the unique needs of the case study, how you will practice integrative case management service delivery.

Advocacy

· Provide “root cause questions for your case

· Explain what advocacy tactic you would employ and why

Ethical Considerations
