In Canada, the emphasis in public health has shifted from the management of communicable diseases to the prevention and management of chronic diseases. One of those chronic diseases that is significant in Canada is heart disease. Heart disease is also known as ischemic heart disease or coronary heart disease. The focus of this assignment is on providing the epidemiological evidence and consequences for heart disease in Canada. Begin this assignment by reading from reputable sources about heart disease in Canada. Reputable sources are sources such as government websites or peer reviewed journals. Please do not use information from newspapers, blogs, Wikipedia or American based information. When you begin your search get in the habit of preparing your reference list immediately by recording all pertinent information on that reference following APA format. If you are struggling to find information, please contact the reference librarian at STFX for assistance in your search. As a STFX student, you have access electronically to many data bases etc. through the STFX library. Give yourself sufficient time to gather information, read it and reflect upon it. Your paper should consist of a cover page that contains vital information such as your name, ID number, course number, date of submission and title of paper. Check your APA manual regarding information that should be on a cover page. If you have headings in your paper, you should have as the second page, a table of contents. This would be followed by the main section of your paper and the last section of the paper is your reference list. The entire paper excluding the cover page, table of contents and reference list should be between 8-9 pages in length. You should have a minimum of 5 reputable and current (5 years old or less) references. All references both in the body of the paper and in the reference list must follow APA format. Be sure to include page numbers in your paper. Start your paper with an introduction, possibly 1-2 paragraphs in length that outlines the purpose and scope of the paper. The main body of the paper is next. This should contain the most current epidemiological data around the rates for heart disease in Canada. Do not get into an explanation of the different types of heart disease as this is not necessary, you already have this information from your previous nursing program. Provide evidence (prevalence and incidence) around where in Canada are the rates highest and who in terms of population groups are most affected by heart disease? Provide epidemiological evidence around what is happening around the trend for heart disease in Canada for the last 10-15 years. How do the rates in Canada compare to other countries such as the United States of America? In the next section of the paper provide evidence around the community based strategies that are being used to address this health issue, include strategies aimed at all levels of prevention. In this section also address how heart disease is changing the landscape for health care in Canada today. The final part of the paper is the conclusion which should be a summarization of the topic. Your paper must be submitted no later than July 5 in the following format. • Use of APA 6th edition that includes a cover page (to include the student’s Last Name, First Name, and Date of Submission.
