Creating a Cover Page

Create a questionnaire based on the information you found while researching. Your Questionnaire MUST include:

• A cover page (1 page maximum)

o Title of study

o Purpose (explain project and what your questionnaire is designed to do)

o Assurance of confidentiality, ask for honest answers

o Where, when, and to whom the questionnaire will be distributed

o Contact information if participant wishes to know results

o Thank you for time and participation

• Background information (less than 1 page)

o Age, sex, grade, education level, etc.

o Only ask if relevant to your study

o Do not ask for real names

o May want to define terms your respondents may not know
