You are assigned to care for Mrs. James in a Long-Term Care facility.  It is time for breakfast and when you try to get her up, she tells you to go away, she is not ready to get up. This is the third time you have attempted to get her up breakfast.

Using techniques for a therapeutic relationship how would respond to Mrs. James. What is the goal of communication in this situation?

Mr. Singh lives in his own home.  You have been caring for him for 3 months, giving him a shower and helping him with breakfast and lunch 3 times a week. He has been getting more forgetful and has wandered away from home several times in the last month.  Today he tells you that his family wants to put him in a Long-Term Care facility.  He is angry about that and claims they just want to get rid of him.

How would you address Mr. Singh in this situation? What aspect of therapeutic communication is important to remember?

You are caring for Juan Fernandez in a retirement home. You notice he is pacing the lounge and muttering to himself. When you ask him, what is wrong he states in an angry tone that this place is boring, there is nothing to do. How would you respond to Mr. Fernandez?

Mrs. Obi is a new resident in the Long-Term Care facility where you are working. She has been blind for the last 10 years. In one paragraph indicate 5 ways you will support a client who is blind. Your answer must be specific to clients who have vision loss.

You have been caring for John Smith, in his home, for 3 months.  Mr. Smith is a diabetic, he weighs 300 lbs. and has a below the knee amputation on his left leg.  When you arrive this morning, he doesn’t call for you to let yourself in as he usually does.  Finding the hidden key, you let yourself in and find Mr. Smith lying on his back on the kitchen floor. He states that he stood up from his wheelchair to get a glass of water, slipped and fell on the floor.  He can’t get up on his own and has been lying there for about 10 minutes. You notice that he is wearing socks, that he has a laceration on his forehead, that is bleeding, and that he can’t move his right arm. He denies feeling dizzy but complains of pain in his right arm.  His BP is 160/76 and P is 88 and regular.

Document your observations and actions taken, if any.

This is the laceration on Mr. Smith’s forehead.
