Consider multi -agency working, communication and collaboration when working with older people with chronic disease.
Task One
Produce an essay which discusses the following;
1) Explain how you can support older people with chronic disease using a multi-agency approach and consider the benefits of this to staff and service user
2) Discuss the importance of collaboration in supporting the service user and explore how to work as an effective team member
3) Reflect on the practitioner’s role in communicating and collaborating effectively with the service user, their families and the care teams
Task Two
1) Provide a written reflection on a critical incident in your work setting where you have demonstrated the ability to transfer good practice to others. Consider how this goodpractice might support service users such as adults with chronic disease.
Formative Feedback
You have the opportunity to submit a draft outline on a word document to demonstrate your planned content and writing style on one of the learning outcomes (maximum 750 words). You should also include 2 citations, 1 direct quotation and references to receive formative feedback.
The feedback is designed to help you develop areas of your work and it helps you develop your skills as an independent learner.
If you are a distance learning student, you should submit your work, by email, to your tutor, no later than 2 weeks before the actual submission deadline. If you are a blended learning student, your tutor will give you a deadline for formative feedback and further details. Formative feedback will not be given to work submitted after the above date or the date specified by your tutor - if a blended learning student.
Assignments submitted late will not be accepted and will be marked as a 0% fail. Your assessment should be submitted as a single Word (MS Word) or PDF file. For more information please see the “Guide to Submitting an Assignment” document available on the module page on iLearn.
