Learning Outcome 1 - Theoretical Frameworks for Care Planning

Title - Care planning (written assessment) - To support this piece of work please include a short summary of your complex care scenario as an appendix. Select an individual in whose care you have participated on clinical placement.

The patient/client must be the focus of your work and each learning outcome must focus on the patient/client. Please anonymise names of people and/or organisations in line with NMC regulations. learning outcomes : 1)

1. Analyse and apply the key theoretical principles and practical skills underpinning the nursing process for assessment to evaluation of care to address peoples' needs.

2. Analyse the skills required for therapeutic relationships across the lifespan

3. Ethically practice compassionate care in accordance with the NMC proficiencies required for the progression to year three of the course

4. Critically reflect upon your personal and professional development in clinical practice. I have attached a document that would be useful

Learning outcome 1 (750 words) -you could discuss:

• The theory behind the different care planning models you are aware. You may wish to consider here the appropriateness of integrated care plans or individualised care plans and what was implemented for your chosen patient.

• How do care plans relate to the nursing process-apply this to the patient, you may wish to consider different Models of Care, for example compare Roper Logan and Tierney to Orem’s model-which one was appropriate to your patient?

• The concept of holism, and the delivery of holistic care

• How was this used to develop and implement a plan of care-think about the biopsychosocial model and the activities of daily living which were affected for the patient

• Discuss the wider team-who were the key team members involved in the care planning process?
Learning outcome 2(750 words)

• Speak about the theory underpinning therapeutic relationships; what about the 6 C’s (You don’t need to list these...)?

• Think about communication theory – read the literature around communication skills for nurses in building therapeutic relationships?

• Consider the quality of life and the impact upon the mental health of your patient and their family/significant others; how has this impacted these relationships?

• Discuss the theory of Family and person - centred care and shared decision making-was it appropriate to plan the care involving family/significant others?  Did you gather information from other people as well as the patient?

• What different forms of communication did you see?  How did you adapt your own communication style to build an effective relationship with the patient and their family/significant others?

Learning outcome 3- (750 words) discuss 

• What ethical considerations are there when undertaking care planning?

•    Think about key aspects such as capacity, consent and confidentiality?

•    What professional considerations are there when care planning – consider the NMC (2018) Code?

•    Was the care delivered appropriate?

•    Were the long- and short-term goals met?

•    What were the strengths of the care plan and delivery and what could have been done differently?

Learning outcome 4- (750 words)

•    Reflect on the care planning scenario you have used.

•    Now you have explored the theories around the care planning process in greater depth- what will you do differently in future?

•    What do you need to build upon to develop your own Clinical Reasoning and Decision - Making capabilities?
