Learning Outcome 1 - Theoretical Frameworks for Care Planning

Title - Care planning (written assessment) - To support this piece of work please include a short summary of your complex care scenario as an appendix. Select an individual in whose care you have participated on clinical placement.

The patient/client must be the focus of your work and each learning outcome must focus on the patient/client. Please anonymise names of people and/or organisations in line with NMC regulations. learning outcomes : 1)

• The theory behind the different care planning models you are aware. You may wish to consider here the appropriateness of integrated care plans or individualised care plans and what was implemented for your chosen patient.


• How do care plans relate to the nursing process-apply this to the patient, you may wish to consider different Models of Care, for example compare Roper Logan and Tierney to Orem’s model-which one was appropriate to your patient?


• The concept of holism, and the delivery of holistic care


• How was this used to develop and implement a plan of care-think about the biopsychosocial model and the activities of daily living which were affected for the patient


• Discuss the wider team-who were the key team members involved in the care planning process?

• Speak about the theory underpinning therapeutic relationships; what about the 6 C’s (You don’t need to list these...)?


• Think about communication theory – read the literature around communication skills for nurses in building therapeutic relationships?


• Consider the quality of life and the impact upon the mental health of your patient and their family/significant others; how has this impacted these relationships?

•    Now you have explored the theories around the care planning process in greater depth- what will you do differently in future?
