Aims:
To explore the impact of psychosocial factors on the health and well-being of a chosen patient/ client To identify and appraise the effectiveness of policies and services aimed at promoting the health and wellbeing of the chosen patient/ client To critically examine the influence of policies aimed at facilitating informed choice and enhancing partnership working.
Choose a patient or client whose care you have been involved in whilst in practice. The patient or client should be an individual whose health and wellbeing has been impacted by psychosocial factors. These may not be the main presenting issues for the client but will have impacted on the client’s health and well-being.Please ensure you discuss your chosen patient/ client with your facilitator before writing the assignment. Introduce the concept of patient choice within contemporary health care, identifying potential barriers/ facilitators that might prevent or enable patients to make choices relating to their health.
Introduce and provide a brief overview of your chosen patient/ client/ family identifying the main presenting factors that have led to their involvement with health care professionals. Please be careful to ensure confidentiality is maintained at all times. A confidentiality statement should be clearly included within the assignment.
