Case Study on a Long-term Condition in Healthcare

Choose a long term condition that you have studied in class or seen on a placement and write a case study on the condition highlighting the following:

• Nursing considerations
• Medication side effects
• Maintenance and management of the condition
• Lifestyle impact of the condition.

You can choose from the long termconditions (including the acute phase) that have been discussed in class or you can write about a patient with a long term condition (including the acute phase) that you have seen whilst on placement. Please refer to the examples discussed in the workshops/seminars to understand the details required for that particular patient you are planning to write about. This assignment should demonstrate your understanding of the condition and what you have learnt during the term

1 Apply knowledge of commonly encountered mental, physical, behavioural and cognitive health conditions. 

2 Apply knowledge of physiology, pathophysiology and pathopsychological of acute and long-term conditions to their assessment and management.   

Thinking Skills

3 Critically analyse and evaluate the effectiveness of a range of evidence-based interventions in acute and community health settings 

Subject-based practical skills

4 Identify and apply a systematic and comprehensive assessment to facilitate the recognition of risk to vulnerable service users, carers and health care professionals and the appropriate response. 

5 Gain relevant experience, skills and knowledge across the 7 platforms and Annexes A and B in the

6 Reflect on their role and the importance of integrated health and social care.

7 Demonstrate the safety implications for medicines management.

In your case study you should:

• Provide a rationale for your choice of topic which should be included in your introduction.
• Critically identify and analyse key epidemiological, statistical, and contextual data/information in relation to the topic.
• Within your written assignment identify and reference the current key supporting health care frameworks, guidelines and strategies relating to your chosen issue.
• Demonstrate your knowledge of physiology, pathophysiology, and pathopsychology of your chosen condition.
• Consider nursing management and additional dimensions in relation to your assessment and management of your chosen condition.
• Provide an understanding of the importance of assessment, planning, implementation, evaluation and communication between the multidisciplinary team to enhance patient care relating to your chosen condition.
• Demonstrate your ability to recognise and interpret with critical understanding the signs of acute deterioration or a change in dependency, in physical or mental health and enabling the appropriate response.
• Identify and apply a systematic and comprehensive assessment to facilitate the recognition of risk to service users, carers and healthcare professionals and the appropriate response.
• Critically analyse and evaluate the effectiveness of a range of evidence-based interventions in acute and community health settings needs to be applied to your chosen condition.
• Consider the role of the nurse in promoting the health awareness of the identified health issue.
• Also consider the wider reputable academic/professional evidence base nationally.

• Consider the role of the nurse in promoting the health awareness of the identified health issue.
• Also consider the wider reputable academic/professional evidence base nationally.
