4NH026 Human Life Journey

The aim of this challenge assignment is for you to demonstrate an appreciation of the human life journey and its diversity from conception to end of life using evidence from a variety of sources.

• A.1. Identify potential stages of human growth and development.

 

• A.3. Demonstrate a systematic method of identifying health care needs.

 

• A.4. Explain how health might be promoted and protected within a given population.

 

1. Demonstrate how to undertake a health needs assessment across the lifespan using evidence based approach (A1 and A3).

 

2. Evaluate current health promotion activities that relate to your practice (A4). In this module you are required to undertake a health needs assessment (HNA) for a local geographical area of your choice.

 

Through this HNA you will develop a geographical profile, exploring factors which impact upon the health of the people within this area.

 

Whilst exploring the public health data you will identify and justify a target population from within the geographical area, along with Two areas of need.

 

After prioritising and justifying one area of need, you will then identify and justify a community health initiative to address this need. The following five steps provide guidance on what you need to do to complete this assignment.

Identify a geographical area local to you. Provide a summary profile for the area; exploring factors impacting upon the health of the people living within this area. Include demographic information relevant to your area (approx 100 words).

Choose a target population within your geographical area based upon a life stage of human development. Identify and justify your choice of target population. Which group of people are of most interest to you? (approx. 250 words) From the data you have collected in your geographical profile identify a target population of people based on a human life stage (e.g. conception; foetus; childhood; adolescence; adulthood; older adulthood). You need to be able to justify your choice of target population in relation to evidence, including national and local data.

Identify and justify Two areas of need and prioritise One. Identify and justify Two areas of need within your chosen target population. Then prioritise One of the two and justify this selection. What is important based upon the evidence you have found exploring your geographical area? (approx. 400 words)

Identify and justify a health promotion initiative using a community-centred approach to improve the health of your target population and address your prioritised health need. What is the aim / purpose of your initiative? How can you help your chosen group? (approx 400 words)

Evaluate. Summarise how the health of the target population will be improved. Consider how this initiative will improve the health of your target population. (approx 200 words).
