You will need at least two tangata whaiora that you can use as case studies for this assignment.
Part A Understanding health and wellbeing
1. Choose two t?ngata whaiora you work with who come from different cultural backgrounds (Note: these may be the same two you use for Part B). Obtain the consent of the t?ngata whaiora for the case studies and ensure anonymity.
2. Talk with each tangata whaiora and ask them:
what is their definition of health and wellbeing
where does their definition come from
how does their definition of health and wellbeing link to how they live their life?
3. Compare and contrast the two different answers you receive to your own understanding of factors that contribute to health and wellbeing. Describe how having this knowledge of the cultural differences helps you work in a person-centered way.
Part B Understanding conditions
1. Choose two t?ngata whaiora you work with who have different conditions. One of these t?ngata whaiora must have co-existing conditions. Obtain the consent of the t?ngata whaiora for the case studies and ensure anonymity.
2. For each tangata whaiora, describe their condition under the following headings:
Description of the condition(s)
Observable or measurable signs and symptoms of condition
Causes and risk factors of condition
Progression of condition.
3. For each tangata whaiora, describe the impacts of their condition(s) on themselves, others, and society.
4. For each tangata whaiora, explain the factors that contribute to their usual state of health and wellbeing.
5. Describe the changes others could make to meet the needs and preferences of each tangata whaiora.
6. Describe how you work with each tangata whaiora in a person-centred way to meet the challenges and impacts of their condition. You must describe at least two aspects of the person-centred approach for each tangata whaiora.
7 . Workplace verification.

For Part B Task 6Â you need to demonstrate that you work in a person-centred way with each tangata whaiora to help meet the challenges and impacts of their condition and have this verified by your workplace verifier.Â
