General Information About the Disease

Write an interview guide that will make sure that you get answers to all the questions in the assignment.

 

Use the occupational profile template as part of your interview in addition to your interview questions. Be aware that using just the occupational profile tool will not be enough to answer all the questions.

 

You will use at least 3 outside resources from peer-reviewed journals to complete information about their chronic conditions and information about how occupational therapy is used with people with their conditions.

 

Assignment: Paper will be in APA format. Paper will be in paragraph form and include the following information. It should be at least 4 pages, double spaced, 11-12 point type, 1 inch margins. The occupational profile and the reference page is in addition to the 4 pages.

 

a. General Information about the disease: (if your client has several chronic conditions, choose that condition that is most impairing to their lifestyle, from their perspective). Give general information about the disease from peer-reviewed journals or from the textbook. This needs to be less than 1 page of your paper.

 

b. Diagnosis: how did the person find out about their disease? What tests were done to determine that they had the disease?

 

c. Medical Treatment: how was the disease treated? What surgeries, therapies and types of medications were used?

 

d. Prognosis: Did the person return to their previous “normal”, or were there permanent physical, cognitive or psychosocial changes associated with this disease? What were they?

 

e. Occupational Impact: What effects did having this disease have on their occupational performance. Include the occupational impact at various stages in the disease, such as: before they were diagnosed, when they were undergoing treatment, as they were recovering, and chronic changes that have occurred.

 

f. Occupational Therapy: Did the person discuss any occupational therapy intervention that they had? If so, what was that intervention, and in what setting(s)? Would they have benefitted from additional OT, if so, in what way? If they did not have occupational therapy, do you think that occupational therapy could have been beneficial to them? How do you think OT could have helped them recover better?

 

g. References: Additional references should be used in any of the sections above as appropriate. For instance, you may use a reference to further define or explain a procedure that the client had as part of their treatment.

 

h. The completed Occupational Profile form

 

i. Any references used, if not direct quotations, should be cited in the text as follows: after the period of the sentence in which you used the reference, put (author last name, year of publication).

 

Example: According to Dr. Gaber, this is the way it should be written. (Gaber, 2019) If you put a direct quotation from a reference, you need to put either quotation marks around the quotation or, for larger quotations, they need to be “inset” paragraphs. Both of these also need to have the citation after (author last name, year of publication, page number).

 

Please consult APA guidelines if you are not sure. Example “You must put in quotations like this.” (Gaber, 2019, p. 103) A reference page needs to be included. It should include at least three references from peer-reviewed journals. Use APA format.
