Descriptive epidemiology report in APA format.  In your report, you need to include the following contents:

1.Format of the paper
a.Title page
b.Table of contents
c.List of Tables
d.List of Figures

2.Background/demographic information (Who) about the State/counties (Where) your groups are in.
a.Include the demographic information summary table you created in GP1 in this report.  Make sure that you have correct title for this table.
b.Describe the background information of counties/state in your group.  Use the social determinants concept to explain the potential public health consequence of the difference in demographic variables (social determinants) between counties?

3.Health Outcomes about the State/counties (Where) your groups are in.
a.Include the Health Outcomes information summary table you created in GP2 in this report. You can  just copy the table from the EXCEL file [Note: make sure that you have corrected the title of the table]
b.Describe the Health Outcomes for counties/state in your group.
i.You need to check whether there is any increase, decrease, or no change of the counties.
c.  Use the social determinants concept to explain the potential reason causes the difference in Health Outcome among counties in your group.
4.Health Factors about the State/counties (Where) your groups are in.
a.Include the Health Factors information summary table you created in GP2 in this report. [Note: make sure that you have corrected the title of the table]
b.Describe the Health Factors for counties/state in your group.
i.Present the overall observation of the Health Factors. You need to check whether there is any increase, decrease, or no change of the counties.
c.  Use the social determinants concept to explain the potential reason cause the difference in Health Factor among counties in your group.

5.Ecological epidemiology data analysis results on the State/counties (Where) your groups are in.
a.Use the regression analysis summary table you created in GP2 to write up this summary report.  No table needs to be included in this report.
b.When you describe the relationship between Health outcome and Health Factors your group selected, you need to include the following information:
i.Use the Regression Line Fit plots to show the relationship (positive, negative, or no relationship) between health factor and health outcome.
a.First present the direction and strength of the association, using r (correlation coefficient)
b.Second, present how much of the health factor yur group selected can predict/explain the health outcome your group selected using r2 (coefficient of determination) including the test results (R2 = value of R square, F(df1, df2) = F value, p = significance F value.  df1:degree of freedom of regression, df2: degree of freedom of residual from the regression analysis ANOVA table)

c.Explain the relationship between the health factor and health outcome in your county. Use the social determinants concept to explain the difference of the association between the health factors and health outcome among your counties.

6.Conclusion of this project.  
a.What have you learned about your home communities from this project?
b.Compare and contrast of the health outcomes and health factors of communities your group are in.
Is there any public health intervention/action you can do for your communities based on the data you collected and analyzed? What is the reason for you to recommend it?
