Capstone Change Project Evaluation Plan
One of the most crucial steps in implementing the evidence-based practice (EBP) proposal is by making certain that the changes we are trying make happen is actually occurs and you have the evidence to prove it. After a proposal has been implemented, it’s important to find a method to gather information to prove that the outcome was achieved and that the data is consistent. As stated in my PICOT question, the outcome is the prevention of falls and related injuries in hospitalized patients through exercise interventions like balance, gait, strength, walking exercises.
 
The evaluation process will include monitoring and assess the reduction in fall rate, related injuries, and recurrent falls after the implementation of the intervention in the follow-up period of eight weeks and author will also evaluate that patient are feeling comfortable with the designed exercise interventions and how many patients are participating in exercise activities. Evaluation process will include the patient’s functional ability and strength to perform their daily routine tasks without being afraid of recurrent falls. Feedback should be obtained from the physicians, nurses, therapist, and patients on this intervention. Resources that are required for implementation of this capstone change project includes computer system, PowerPoint presentation, information, financial resources, human resources, and clinical resources.
 
Assessment of the developed interventions is very important to decide the effectiveness of the change project. This project will be evaluated by using summative evaluation methods (Jonathan, 2017). By using this summative method, the outcomes, or changes brought by this change project will be assessed. The evaluation method may compare the number of fall incidences before and after the project implementation. The evaluators can also compare the health outcomes of the elderly patient before and after the project implementation. Finally, author will evaluate the need for improvement in the designed exercise activity and time period.
