2. Your project plan should also include a measuring/reporting strategy. Measurement and reporting strategy should include:

a. how you will measure success

d. when will reports be produced (weekly, fortnightly, monthly)

e. who will receive a copy of the report?

f. Your project plan should also include a list of resources that is needed to implement the changes. (tools, supplies, etc.)

3. Indicate how you will identify and respond to barriers to the change

4. Include how you will action interventions and activities set out in the project plan.

5. Explain how what strategies you will use to embed the changes set out in your plan.

6. Provide information about how you plan to conduct regular evaluations and reviews of the project plan to achieve change program objectives.
